The GDPR route

A recording is personal data under the GDPR, so your firm also needs a lawful basis for the processing. That note belongs in your file just as much as the report itself does.
In practice it comes down to three things: you say that you're recording and what for, you give the client a real say, and you note that you did. Would the client rather not? Then you fall back on written notes.
